PRE-VISIT INSTRUCTIONS FOR ROUTINE EYE EXAM POLICY
Eyeglasses and Contact Lens Fitting Policy at Eye Florida
For your upcoming ROUTINE EYE EXAM, we require that these PRE-VISIT INSTRUCTIONS are followed in order to prevent delays in your eye care and prevent unnecessary follow-ups. We strive to provide quality and timely eye care. Understanding and complying with these instructions will help you plan your day and have a better experience at our office.
CONTACT LENSES NEW FIT OR RE-FIT:
If you are currently wearing contact lenses, you must STOP wearing your contact lenses at least 5-7 days prior to your eye appointment. Long-term contact lens wear, including wearing your contact lenses while sleeping, can cause conditions such as “corneal warpage” and “keratitis”. This will result in inaccurate measurements of your glasses prescription at the time of your visits. If you are unable to stop wearing your contact lenses, our office will not be responsible for glasses re-do. There will be no refunds for your glasses purchase and you will be responsible for any additional co-pays and office visit fees for any additional visits. Your vision exam benefits only covers 1 visit per year. Please review your vision insurance benefits prior to your visit.
Please bring your contact lenses in it’s original cases (contact lens boxes with readable prescription, base curve (B.C.) and diameter (DIA). If you do not have your contact lenses in its original boxes, we will NOT be able to fit you with contact lenses on the day of your visit. We will obtain corneal measurements (corneal topography) and will order your contact trials. Contact trials can take 2-6 weeks for delivery to our office. If you need your contact lenses right away, we recommend you schedule your appointment with a retail optometry/optical/contact-lens specific office that can meet your needs. Our office is an ophthalmology practice and we offer contact lens fitting service but we do NOT have contact lens trials ready and available for same day fitting.
If you require expedited order, there will be an expedited fee. You are required to pay the full fee prior to any order being placed.
Your contact lens prescription is valid for one-year from our office. Some offices offer a 2 year on your contact lenses prescription. We will not re-fill your contact lens prescriptions if it is over a year and you are unable to come in for an eye exam due to medical-legal concerns.
GLASSES CHECK AND UPDATE IN GLASSES PRESCRIPTION:
Please bring your glasses to your visit.
Plan to have your eyes DILATED. You may need to bring a driver. Your eye visits may take 1-2 hours depending on how long it will take to fully dilate your eyes for exam. If you are in a time constraint, we recommend you schedule your appointment first thing in the morning or in the afternoon. Another options is to schedule your appointment with another office that can accommodate your schedule.
If you are wearing contact lenses, please STOP wearing your contact lenses at least 5-7 days prior to your eye appointment. Refer to #1 under CONTACT LENSES NEW FIT OR RE-FIT.
If you are unable to stop wearing your contact lenses, our office will not be responsible for glasses re-do. There will be no refunds for your glasses purchase and you will be responsible for any additional co-pays and office visit fees for any additional visits. Your vision exam benefits only covers 1 visit per year and may only cover a contact lens fitting exam or a glasses exam, not both. Please review your vision insurance benefits prior to your visit.
We are here to help you with your eye care including contact lenses fitting and glasses update. If you are unable to follow these PRE-VISIT INSTRUCTIONS, Eye Florida will not be responsible for any of your additional co-pays and office visit fees for any additional visits. If you have a complaint about your experience at Eye Florida, we request that you speak to our office administrator first. We request that you do not post your comments on social media including Google Review and Yelp until you speak to our office administrator.
